• Basic Life Support/Advanced Life Support Certificate
• DBS Certificate (issued in last 3 years)
• Certificate of Completion of Training (CCT/JCPTGP) Certificate
• Medical Performers List letter of approval
• Document Evidence of Immunity against Hep B – Antibodies (Titre Levels)
